ABSTRACT

Radical resection of gastrointestinal tumors based on the membrane anatomy theory has significantly reduced the postoperative recurrence rate and improved the surgical efficacy. However, the theory of membrane anatomy has not been widely adopted in radical surgery for esophageal cancer. Our study found that the esophagus also has a membranous anatomical structure. As a foregut organ, the esophagus also has a mesenteric structure, and there is also a fifth metastasis pathway within the esophageal mesentery for esophageal cancers. The leak and metastasis of cancer caused by destruction of the mesenteric integrity may be the fundamental reason for the high postoperative recurrence rate. Using the nano carbon and indocyanine green fluorescence tracing technique, we demonstrated the lymphatic drainage of the upper esophageal segment to the left gastric artery mesenteric lymph nodes. Therefore, in the radical resection of esophageal cancer, we used the membrane anatomy theory for guidance to completely remove the esophageal cancer, esophageal mesentery, left gastric artery and its mesentery, as well as all structures within the mesentery, preventing the spread of cancer cells through the blood vessels, lymphatic system, and mesentery, and improving the efficacy and prognosis. This article elaborates on the theoretical basis of the anatomical structure of the esophageal membrane, embryonic development, imaging, autopsy, and endoscopic observation of the structure, as well as the application effect of the esophageal membrane anatomical theory in esophageal cancer radical surgery. It elucidates the anatomical structure of the esophageal membrane and the lymphatic drainage characteristics of esophageal cancer, reveals the law of lymphatic metastasis in esophageal cancer, optimizes lymphatic dissection strategies, and improves the efficacy of esophageal cancer radical surgery.

ABSTRACT

Glycosylation, one of the most common and important reactions in biological systems, results in diverse functions and is often found in biologically active small-molecule natural products produced by microorganisms. Furthermore, sugar moieties are generally critical for their activities. Alternating the sugar structures thus provides the potentials for enhancing the biological activities of natural products, which evokes researchers to study the sugar biosynthetic machinery and its application in the modification of sugar moieties with an aim of generating unnaturally glycosylated natural product drugs with better activities. This review will briefly outline current studies on sugar biosynthesis and glycosyltransferase, with a few selected experiments designed to alter natural-product sugar structures.

ABSTRACT

Objective To investigate the space structure of plague natural foci in the area of Lantsang, Yellow and Yangtse River in Qinghai Province to provide references for making decisions to eontrol the occurrence of human plague. Methods Data was collected from the survey on natural foci and surveillance of plague from 1954 to 2006 and descriptive epidemiological method was used to analyze the data. Results Marmata hirnalayana and Microtus fuscua natural foci were known in Sanjiangyuan area. Callopsylla dolabris, Oropsylla silantiewi, Citellophilus sparsilis and Amphipsylla tuta were vectors; Microtus fuscus plague natural foci was in a range of about 9500 km2, distributing in Zhenqin Town, Chengduo County. Marmata himalayana plague natural foci distributed over 13 countries, a range nearly 107 000 km2. By the end of 2006, 450 strains of Yersinia pestis were detected and separated from 6 kinds of rodents, 6 kinds of carnivora, 3 kinds of artiodactyls and 9 insects vectors. Between 1960 and 2006, 238 cases and 134 deaths from plague were reported. Most human plague cases occurred in the months from May to November and usually presented as one of three primary forms-bubonic 17.23%(41/238), septicemic 16.81% (40/238), pneumonic 61.34% (146/238) and other types 4.62% (11/238). However, the first epidemic plague case was mainly the glandular plague. Conclusions Date suggested that plague is still a critical public health problem in Sanjiangyuan area, against which countermaeasure needs to be strengthened in the main epidemic areas. More scientific researches on plague should be carried out. Surveillance networks of reporting suspected plague have been established and reduce the number of human plague cases.

ABSTRACT

OBJECTIVETo study the interaction between Toll-like receptor (TLR) 4 and myeloid differentiation protein-2 (MD-2) in living cells using fluorescence resonance energy transfer (FRET) technology.

METHODSThe coding sequences of TLR4 and MD-2 (without the signal peptide sequence) were amplified by PCR and cloned into enhanced cyan fluorescence protein (CFP) and enhanced yellow fluorescence protein (YFP) expression vectors carrying TLR4 signal peptides (pECFP-C1-SP and pEYFP-C1-SP). HEK293 cells were transfected respectively or together with the reconstructed plasmids verified by enzyme digestion and sequence analysis, and the expression and sublocalization of these fluorescence proteins in the cells were observed using fluorescence microscope. FRET in the cells coexpressing CFP-TLR4 and YFP-MD-2 was detected using routine and acceptor photobleaching method.

RESULTSThe reconstructed plasmids were expressed in HEK293 cells. The cyan or yellow fluorescence was located in the cytoplasm, mainly around the nucleus in the cells transfected with pECFP/TLR4 or pEYFP/MD-2, and both the cyan and yellow fluorescence located mainly in the membrane and occasional in the cytoplasm of cells cotransfected with pECFP/TLR4 and pEYFP/MD-2. Routine or acceptor photobleaching detected FRET phenomena in cells coexpressing CFP-TLR4 and YFP-MD-2, suggesting direct interaction between TLR4 and MD-2.

CONCLUSIONThis study provides direct evidence of the interaction between TLR4 and MD-2 in living cells.
